Treatment Types

Partial Hospitalization Programs (PHP) provide the most intensive outpatient-level treatment available — typically 20 to 30+ hours per week across 5 to 7 days. Patients attend full-day programming at a treatment facility but return home or to a sober living residence each evening.

Structure and Components

A typical PHP day runs 5-6 hours and includes individual therapy, group therapy (process groups, psychoeducation, skill-building), psychiatric evaluation and medication management, nursing assessments and vital sign monitoring, experiential therapies (art, music, movement), and family sessions (usually weekly).

PHP provides a level of medical monitoring that standard outpatient cannot — nurses and psychiatrists are available during treatment hours, medications can be adjusted in real-time, and symptoms can be assessed daily. This makes PHP particularly suitable for patients transitioning from inpatient care or those who need close monitoring but don't require 24/7 supervision.

When PHP Is Appropriate

ASAM Level 2.5 (PHP) is indicated for patients who are medically stable but need daily monitoring, have completed detox but require intensive therapeutic intervention, are stepping down from residential treatment and need continued structure, have co-occurring mental health conditions requiring psychiatric oversight, and need more than IOP but cannot commit to residential care.

PHP vs. Residential Treatment

The main differences: residential provides 24/7 supervision and a controlled environment, while PHP allows patients to return home or to sober living. PHP costs less than residential treatment and allows patients to maintain some daily responsibilities. However, PHP requires a safe living environment — patients returning to a home with active substance use or domestic instability are better served by residential care.

Duration and Transition

PHP programs typically last 2-4 weeks before stepping down to IOP. The transition is based on clinical progress: stable mood, reduced cravings, improved coping skills, and a clear aftercare plan. Many treatment centers offer a continuum from PHP to IOP to standard outpatient within the same program, providing consistency of care.

Effectiveness

Research supports PHP as a cost-effective alternative to residential treatment for many patients. A study in Psychiatric Services found no significant differences in clinical outcomes between PHP and inpatient treatment for patients meeting appropriate clinical criteria, while PHP costs were approximately 30-50% lower.
